Transportation Service Agreement
Branch Transportation LLC
This Transportation Service Agreement (“Agreement”) is entered into between Branch Transportation LLC (“Company”) and the undersigned client (“Client”) for the purpose of providing non-emergency transportation services.
1. Services Provided
Branch Transportation LLC provides non-emergency transportation services to individuals including seniors, patients, and passengers requiring transportation to medical appointments, facilities, errands, or other approved destinations.
Drivers may provide basic walking assistance to and from the vehicle or building entrance when requested.
Drivers are not medical professionals and are not permitted to provide medical care, physical lifting, or mobility transfers.
2. Scheduling
All transportation services must be scheduled in advance.
Same-day requests will be accommodated only when availability permits.
Clients must provide accurate pickup and drop-off information at the time of booking.
3. Payment Terms
Payment for services is due at the time of service, unless prior billing arrangements have been approved by Branch Transportation LLC.
Accepted payment methods may include:
• Cash
• Electronic payment (Cash App, Zelle, etc.)
• Invoice billing (if pre-approved)
Failure to make payment may result in suspension or refusal of future services.
4. Cancellation and No-Show Policy
Clients must cancel scheduled rides at least 24 hours prior to the pickup time.
The following may result in fees:
• Late cancellations (less than 24 hours notice)
• No-shows
• Passenger not ready at pickup time
5. Waiting Time
Drivers will wait up to 15 minutes from the scheduled pickup time.
Additional waiting time may be subject to additional charges or ride cancellation.
6. Passenger Safety and Conduct
For the safety of all passengers and drivers, clients must:
• Wear seat belts at all times
• Follow driver instructions
• Treat drivers and vehicles respectfully
Branch Transportation LLC reserves the right to refuse or terminate service if a passenger is intoxicated, abusive, or creates an unsafe environment.
7. Delays
Branch Transportation LLC will make every effort to provide timely transportation, however the company is not responsible for delays caused by:
• Traffic conditions
• Weather
• Road closures
• Mechanical issues
• Other unforeseen circumstances
8. Personal Belongings
Branch Transportation LLC is not responsible for lost, stolen, or damaged personal belongings left in the vehicle.
9. Liability
Branch Transportation LLC is a non-emergency transportation provider and carries required insurance coverage.
By using this service, the client acknowledges that the company is not responsible for injuries or incidents occurring outside the scope of normal transportation services.
10. Service Refusal
Branch Transportation LLC reserves the right to refuse or discontinue service if safety concerns arise or if terms of this agreement are violated.
11. Agreement Acceptance
By signing below, the client acknowledges that they have read, understood, and agree to the terms and conditions outlined in this Transportation Service Agreement.
